OBJECTIVE

  • To provide manpower and facilities for the efficient and unobtrusive transportation, storage and handling of cadavers within the hospital.
  • To provide consultation, manpower and facilities for the performance of post mortem.
  • To provide services for Crime Scene Investigation (CSI) and exhumation.
  • To provide Post Mortem report for Police Officers and official claimant when requested.
  • To provide Mass Fatalities Management and Disaster Victims Identification (DVI).
  • To provide premise for performing tissue procurement process for Brought in Dead (BID) and Death in Department (DID) cases.
  • To provide consultation in managing and examination of patient for clinical forensic cases.
  • To provide specimen handling services in all medico legal autopsy cases.
  • To provide specimen handling services in all clinical forensic cases.
  • To provide laboratory services for forensic cases.
  • To provide testimony in court as professional witness upon court order.
  • To assist National Suicide Registry Malaysia (NSRM) in providing relevant data.

SCOPE OF SERVICE

Pathology Forensic Unit

  • Providing service of transporting deceased from ward to mortuary.
  • Providing temporary storage space (for immediate body claiming) and body freezer (for delayed body claiming).
  • Documentation of details of deceased into record, including FMIS (Forensic Medicine Information System).
  • Assisting in finding next-of-kin of unknown body.
  • Providing premise for conducting last office for the deceased.
  • Providing post mortem service for medico legal cases.
  • Providing post mortem service for high risk cases.
  • Providing consultation for post mortem cases from district hospital.
  • Producing Post Mortem report upon request.
  • Handling of medico legal specimen for post mortem cases.
  • Conducting Crime Scene Investigation (CSI) and preliminary body examination at the scene.  
  • Medico legal exhumation as well as welfare services in supervising exhumation upon request from Majlis Ugama Islam Pahang and NGOs in Pahang
  • Managing mass fatalities at state and national level, including facilities, manpower, scene management, body tagging/storage/documentation/release and specimen handling
  • Conducting the process of Disaster Victims Identification (DVI).
  • Providing premise for Hospital Tissue/Organ Procurement (TOP) team in conducting procedure of tissue procurement from Brought in Dead (BID) and Death in Department (DID) cases.
  • Providing premise for next-of-kin(s)/relative(s) counselling by Assistant Environmental Health Officer for high risk cases.
  • Assisting NSRM in collecting data and statistics for suicide cases in Pahang.

Clinical Forensic Unit

  • Handling of medico legal specimen for clinical forensic cases.
  • Giving consultation for clinical forensic cases when referred.

Laboratory of Forensic Medicine

  • Providing Forensic Histopathology Laboratory service in producing slides for analysis.
  • Providing service of examination and reporting of Forensic Histopathological slides for autopsy cases in Pahang.
  • Providing Forensic Anthropology Laboratory service including processing and cleaning; examination and reporting.
  • Providing Forensic Diatom Analysis service.

POLICY DESCRIPTION

  • Pathology Forensic Unit
    • Body handling for normal cases without post mortem
      • Patients who die in hospital will be transported to mortuary using cadaver trolley. This should be carried out by two Health Attendants, one from Department of Forensic Medicine and one from the ward.
      • The body will be placed either at the temporary storage space (for immediate release) or body freezer (for delayed release).
      • The body can be released once all the relevant documents are completed.
      • Department of Forensic Medicine will not provide the service of last office or hearse.
      • For unknown body and unclaimed body, Assistant of Medical Officer (AMO) will take further action according to the guidelines.
  • Body handling for post mortem cases
    • Body for post mortem will be kept in the body freezer awaiting for autopsy.
    • Post mortem is mandatory on the written order of the police, i.e. P61 and shall be conducted as soon as practicable once P61 issued (within 24 hours for non-complicated case).
    • Only assigned staff are allowed to attend post mortem.
    • The body is released as usual once post mortem is completed and all the relevant documents are completed.
    • For unknown body and unclaimed body, Assistant of Medical Officer (AMO) will take further action according to the guidelines.
  • Body handling for infectious cases without post mortem
    • Infected body will be transferred from ward to mortuary using cadaver trolley. This should be carried out by two Health Attendants, one from Department of Forensic Medicine and one from the ward. Full PPE will be put on if necessary.
    • Assistant Environmental Health Officer shall be informed.
    • The body will be placed either at the temporary storage space (for immediate release) or body freezer (for delayed release).
    • The body can be released once all the relevant documents are completed.
    • Department of Forensic Medicine will not provide the service of last office or hearse.
    • For unknown body and unclaimed body, Assistant of Medical Officer (AMO) will take further action according to the guidelines.
  • Body handling for infectious post mortem cases
    • Body for post mortem will be kept in the body freezer awaiting for autopsy. Assistant Environmental Health Officer shall be informed.
    • Post mortem is mandatory on the written order of the police, i.e. P61 or that of the Director of General of Ministry Of Health (under the the act of Disease – Bearing Insect Act 1975 and Prevention And Control of Infections Disease Act 1988) and shall be conducted as soon as practicable once P61 issued (within 24 hours for non-complicated case).
    • Head of Department will giving instruction if the post mortem shall be carried out in the high risk autopsy room.
    • Only assigned staff are allowed to attend post mortem.
    • Level of PPE used during post mortem is dependant on the type of infection.
    • The body is released as usual once post mortem is completed and all the relevant documents are completed.
    • For unknown body and unclaimed body, Assistant of Medical Officer (AMO) will take further action according to the guidelines.
  • Mass fatalities management and DVI
    • If the number of fatalities is within the capability of Department of Forensic Medicine, the operation instruction shall be conducted by Head of Department, Department of Forensic Medicine.
    • If the number of fatalities is exceed the capability of Department of Forensic Medicine, , the operation instruction shall be conducted by National Institute of Forensic Medicine.
    • The entire staff will be involved in the process of Ante Mortem and Post Mortem for DVI.
  • Tissue procurement
    • Forensic Pathology Unit assists in informing TOP team if there is any potential donor.
    • Forensic Pathology Unit provides premise for TOP to conduct the procurement process.
  • Clinical Forensic Unit
    • Department of Forensic Medicine offers staff in handling specimen for clinical forensic cases.
    • Giving consultation for examination when case is referred.
  • Laboratory of Forensic Medicine
    • Forensic Histopathology Service
      • Provide the service of processing autopsy’s organ/tissue sent into slides.
      • Provide the service of examining the slides followed by issuing the report.
  • Forensic Anthropology Service
    • Providing the service of processing and cleaning the specimen.
    • Provide examination of the specimen and report.
  • Forensic Diatom Analysis
    • Providing the service of processing autopsy’s organ/tissue and water sample sent into slides.
    • Provide the service of examining the slides followed by issuing the report.
